| Description | The VQF form just has a checkbox there to allow the signer to check a box that the ID document is indeed attached. So there should be no entry-line "Identification document". I also think "Picture of identification document" should be replace with "Copy of identification document". The same applies to "Company identification document" and any of the other "Identification document" inputs. | ||||

What I understand from the field "identification document" is that there is text field and a checkbox. The text field is for the officer / customer to enter the identification document NUMBER or TEXT by the fact that I can enter text in the field. In the case of the SPA, the text is allowed to be entered in one field and there is no checkbox since the customer/officer upload a document directly. I'm not sure if you want to remove the TEXT field and only leave the "Copy of" upload field for all the cases. Seems that is not what the VQF document allows. |

I think you're mis-reading the form. There is no text field in the form -- it is really JUST the checkbox, and the checkbox is there to check that there is an 'appendix'. So you ONLY need to have an upload possibility here, and neither a checkbox nor a text field. |
